2024 GFL PHYSICAL FORM

All football players and cheerleaders must complete the GFL 2024 Physical Form to become a certified participant.
It must be on the “Season Year: 2024” Form in order to be accepted.
Examination/physical must be performed in 2024 and completed by a MD, DO, NP, or PA only.
A signed/completed 2024 GFL Physical Form is required to be submitted to your LAA Cheer Board for all cheerleaders prior to participating in any GFL activity.
No photocopies will be accepted. Originals only and EVERY line on the form must be completed. No exceptions!
